Sovereign Hospice provides un-interrupted hospice services on a 24 hour basis. You can count on us any time you need us regardless of date, day or time. We have physicians, registered nurses, social workers and chaplains on call round the clock to meet any of your needs and will make home visits as needed and as requested by the patient or family. Sovereign hospice also offers 24 hour pharmacy services, which means the hospice physician can order any medication and it will be delivered to your door step within a matter of hours. The same goes for Durable Medical Equipment (DME) or Medical supplies such as (underwear/diapers/chux/gloves/underpads). Patient needs are met on a 24 hour basis.

Do Not Call 911 if you are enrolled in our hospice program. We are your 911. Our registered nurses can make home visits 24 hours a day. Our triage nurse will stay with you on the phone while the on call nurse is en-route to your residence. If the need arises, we an put a nurse at your bedside 24 hours a day for crisis management. We can do pretty much everything a hospital could do for a dying patient that calls 911. Our goal is to prevent hospitalizations and allow natural death of terminal patients at home.

As a patient deteriorates the hospice nurse will visit several times daily round the clock followed by hospice social worker and hospice chaplain to ensure that family is well supported during the difficult time. If a patient has uncontrolled symptoms we will put a nurse by the beside 24 hours until the symptoms can resolve or until the patient passes. The level of care need is assessed on a day to day basis by the RN Case Manager.

We offer Respite care for up to 5 days at a Medicare Certified Nursing Home and cover all associated costs including transport should the family or caregiver need a break. The hospice team will make daily visits while the patient is in respite care temporarily at a nursing home to relieve family.

Sovereign Hospice offers specialized Inpatient Hospice Care at partnering acute care hospitals if the hospice team determine that symptoms cannot be managed in a home setting and will pay for the hospitalization and all related costs. Generally this level of care is required for patient coming off ventilator, extreme pain/dyspnea needing drip medications or other symptoms that cannot be managed in a lower level of care. Maximum expectancy of a patient to stay in this level of care is 24 – 48 hours.
